1. Right-click on the link above and select "Save Target As..."
   
   
2. A "Save As..." dialog appears for you to select
   Save in: (location on machine)
   File name: (keep default or rename file)
   Save as type: (will save the file as an Adobe Acrobat file)
   
   
3. Then click on "Save" button and now the file is saved locally on your machine!
   
   
4. Open the file with Adobe Reader or Acrobat and the click on the Typewriter icon;
   and now the cursor is enabled for interactively filling in the form!
   Just click anywhere in the form and begin typing
   (re-click the Typewriter icon, after moving the cursor, to re-enable the tool!)
   
   
5. Save your changes, print and mail form to the address on the form
