Right-click on the link above and select "Save Target As..."
A "Save As..." dialog appears for you to select:
a. Save in: (location on machine)
b. File name: (keep default or rename file)
c. Save as type: (will save the file as an Adobe Acrobat file)
Click on "Save" button to save the file locally on your computer!
Double-click file (*.pdf files are associated with Adobe Acrobat or Reader)
a. Click the Typewriter icon
b. Cursor is enabled to interactively fill out the form
(Just click anywhere in the form and begin typing)
c. Click the Typewriter icon to re-enable the Typewriter tool!
Save the form (if you wish)
a. Print and
b. Mail form to the address listed within
